Our Housing Transition Services help individuals and families plan for, find, and move into a safe, stable home. We assist with:

• Identifying housing options based on your needs and preferences.
• Navigating rental applications, lease agreements, and landlord communications.
• Moving and setting up your new home to ensure comfort and security.

We work closely with each client to ensure a smooth transition into a home where you can begin building a better future.

Once you’ve secured a home, Housing Sustaining Services offer ongoing support to help you maintain stable housing. These services include:

• Budgeting assistance and financial planning to ensure you can meet your rent and utilities.
• Connecting you with community resources and benefits, such as food assistance and healthcare.
• Ongoing emotional and practical support to prevent eviction and address challenges you may face.

We are here to help you sustain your housing, giving you the tools and support you need to thrive in your new home.

If you do not have Medical Assistance (MA) case management, our Housing Consulting Services offer a person-centered approach to create a customized housing plan. This service is designed to help individuals who need support in planning and navigating their housing needs. Our housing consultants assist with:

• Creating a personalized housing plan tailored to your unique situation.
• Identifying potential housing solutions and support options.
• Providing advice and guidance throughout the entire process.

Our consultants work closely with clients to ensure that their housing journey is successful and sustainable.

You may qualify for Housing Stabilization Services if you meet the following criteria:

• You have Medical Assistance (MA).
• You are 18 or older.
• You have a disability or disabling condition.

Additionally, you should be in one of the following situations:

• Homeless or at risk of homelessness.
• Moving out of an institution, nursing facility, or certain other group settings.
• At risk of institutionalization.

You must also need assistance with at least one of the following areas:

• Communication
• Mobility
• Decision-making
• Managing moods or behaviors
